Gym Membership Reimbursement Form

ThriveOn offers a gym membership reimbursement program to encourage you to take steps to improve your health. Physical activity is an important part of overall health and well-being: we want to help you get and stay fit!

Who is eligible to submit for reimbursement?
All benefits-eligible employees and benefits-enrolled spouses/domestic partners in the Franklin County Cooperative are eligible to receive reimbursement. One person per household eligible per month except households with two eligible employees (i.e- spouses or parent/dependent child).

How do I apply for a gym reimbursement?
To qualify, you must provide proof of payment and proof of attendance at your gym. Please ensure your name is listed on both your proof of payment and proof of attendance. You will only be reimbursed for those months you can verify payment and activity.

How much will be reimbursed? How often do I have to go to the gym?
How often you use your membership determines how much you will be reimbursed. It must be the same person visiting the gym each time during the month. We do not reimburse annual fees.
0-3 days per month - no reimbursement
4-7 days per month - 50% of monthly membership cost, up to $25/month
8 or more days per month - 100% of monthly membership cost, up to $50/month
You can combine visits from two gyms. Please note, you must show proof of payment and proof of attendance for both gyms.

What if I miss the submission deadline?
We will not extend any deadlines. If you do not submit your form by the deadline for that quarter, you will miss out on the reimbursement opportunity. The online submission form closes at 11:59 p.m. on the submission deadline date.

The 2025 submission deadlines and payouts are as follows:

What types of gym expenses are eligible for reimbursement?

  • Full-service gyms such as: LA Fitness, Planet Fitness, Snap Fitness, Crunch Fitness, YMCA, local recreation centers, etc.
  • Boutique-style gyms or studios including Jazzercise, Curves, Orange Theory, Yoga, Kickboxing, etc.
  • Personal trainers
  • At-home/virtual memberships such as Peloton, iFIT, etc.

Note: all memberships, including at-home memberships and personal trainers, are subject to the same requirements listed above (proof of visits, proof of payment).

How/when will reimbursement be paid to me?
Gym reimbursement payments will be made on a quarterly basis. Your reimbursement will be processed through payroll and included as additional taxable income on your paycheck. Please keep in mind the following criteria for issuing incentive payouts:
1. ThriveOn cannot issue incentive payouts to anyone who is not in an active payroll status.
2. Incentive payouts are made on a pre-determined schedule throughout the year. ThriveOn cannot issue incentive payouts ahead of schedule. (See table above for this year's payout dates)
